Accomplishment of Health and Travel History Declaration Form (We will provide for you to fill out) 2. FOR INTERNATIONAL GUESTS: Copy of COVID-19 negative test results within 14 days for Guest/s or Visitor/s with international travel history for the past 14 days. The condo office is closed after Noon on Saturday and will not be open until 9 am on Monday morning.
